Forms

Applications, orders, contact: one button, your questions, answers sent to staff.

An application in DMs, an order jotted in a random channel, a request sent to three people: nothing’s in one place and things slip through. Forms adds a button and your questions, then gathers every answer in a single channel for your team, each with two buttons: Accept or Decline.

What it’s for

01

Ask your questions

Up to 5 questions: short answer, long answer, multiple choice or file upload. Members fill in a small window without leaving Discord.

02

One click to decide

Accept or Decline under every answer. On accept, the role you picked is given automatically and the member is notified.

03

A tracking board

Who’s waiting, who’s approved, who’s declined. A pinned message in the channel that updates itself with every decision.

How it works

  1. Create your form

Write the title, text and questions. The button is set up right under the preview: its text, emoji and color.

  2. Pick the two channels

Where the button appears, and where answers arrive for your team. Then click Deploy.

  3. The member fills it in

They click the button and a small window opens with your questions. Their answer goes to your team’s channel, with their name and avatar.

  4. Your team accepts or declines

Two buttons under every answer. The tracking board updates itself, no extra steps.

Settings

Panel
The title, text, banner and color of the message carrying the button. Same editor as Ultiz’s other panels.
Button
Its text, emoji and color. Set right under the panel preview.
Panel channel
Where the button is shown, for members.
Inbox channel
Where answers arrive, for your team.
Role given on accept
When your team clicks Accept, this role is given to the member. You can leave it empty.
Questions
5 max. For each: the question, helper text shown in the field, and required or optional.
Answer type
Short answer, long answer, multiple choice (2 to 25 options) or file upload (up to 10). For the first two, you can set a minimum and maximum length.
Tracking board
A toggle at the bottom of the form. On, your team sees a message counting pending, approved and declined requests.
Board title
What your team reads at the top of the board. 40 characters.
Board channel
The inbox channel by default, so the board sits next to the requests. You can put it elsewhere.
Columns
The three words of your trade, each with its color dot. A designer might write To do, In progress, Delivered.
Always at the bottom
The bot reposts the board under the latest message so it stays in view. Avoid on a very busy channel.
/suivi command
Your team types the command anywhere and sees the board, visible only to them.
Nudge what’s lagging
One reminder a day in the channel if requests have been waiting longer than the number of hours you set. You can have a role pinged.
Named requests under the counters
How many pending requests are listed by name under the numbers. 10 max. Set 0 to show only the numbers.

What your members see

  • Fill in the form

The panel button. The member clicks and the question window opens.

  • Accept or Decline

The two buttons under every answer, reserved for your team.

  • The magnifier

On the right of a board line. Jumps straight to that request’s message.

  • See all

Under the board. Shows the full list, visible only to whoever clicks.

  • /suivi

Your team types it anywhere and sees their boards, visible only to them. The /suivi command option must be turned on in the board.

Good to know

  • Accept and Decline are reserved for your team: the click only works with the Manage Roles permission, or as an administrator.

  • A form without questions is useless: the member clicks and nothing opens. The Deploy button stays locked until there’s at least one question and every question has a name.

  • The reply DM only arrives if the member accepts direct messages. The role is given even if the message doesn’t get through.
